Why We Require Background Checks
To ensure the safety and security of all our members, The Center of Sarasota conducts background checks on all membership applicants. This helps us maintain a safe, trustworthy community.
Authorization and Consent
By signing this authorization, I hereby voluntarily authorize The Center of Sarasota and its designated agents to conduct a comprehensive background check, which may include but is not limited to:
- Criminal history records check (federal, state, and county)
- Sex offender registry check
- Identity verification
- Public records search
Disclosure and Acknowledgment
I understand and acknowledge that:
- The background check will be conducted by a third-party consumer reporting agency
- I have the right to request a copy of the background check report
- I have the right to dispute any inaccurate information in the report
- The club may deny membership based on the results of the background check
- Information obtained will be kept confidential and used solely for membership evaluation
- The background check may take 3-7 business days to complete
Certification
I certify that all information I have provided is true, accurate, and complete. I understand that any false or misleading information may result in denial or termination of membership. I release The Center of Sarasota, its employees, and agents from any liability in connection with the background check process.
Fair Credit Reporting Act (FCRA) Rights
Under the Fair Credit Reporting Act (FCRA), you have the right to:
- Know if information in your file has been used against you
- Know what is in your file
- Ask for your credit score
- Dispute incomplete or inaccurate information
- Limit “prescreened” offers of credit and insurance
